Physical Requirements and Athletic Forms

There is a Physical Requirement for all students in the 6th, 7th and 8th grade who will be participating in any of the sports programs.

The New Jersey Department of Education requires that all students in the above grades have a pre-participation Sports Physical before the first day of practice for the designated sport. This physical form must be completed and signed by the parent and physician using the required form. Please return the physical form to the nurse’s office.

In addition to the athletic physical form, please download and read the Concussion Policy Pamphlet, the Sudden Cardiac Death in Young Athletes Pamphlet and the Sports Related Eye Injury Pamphlet. Do not sign those pamphlets. Keep them for reference. You and your child need to sign the Parental Sign-Off Sheet and return it to your child’s coach.

This information is part of the Scholastic Student-Athlete Safety Act, P.L.2013, c.71

Please call the nurse’s office (856-665-7121) if you have any questions.
